Upgrade to Surgery telephone system
Quantock Vale surgery has installed a new telephone system to help patients contact the appropriate person to help with their enquiries. This will be in the form of a telephone menu system. Please click here for more information.
The telephone prescription line times will be changing from 4th June 2018
The telephone prescription line currently runs from 10am to 12 noon and 2pm to 4pm. From Monday 4th of June, the prescription line will be open from 9.30 am to 12 noon only. The morning session has been extended but there will be no afternoon service. This is in response to patient feedback and also the successful uptake of the online patient access system (which allows patient to put in their repeat medication requests in 24 hours a day). We recommend patients to use this service when possible. Please click on the link on the main webpage for registration details.
PPG fundraising cake and coffee morning 2017
A big thank you to all the staff, patients and members of the PPG who were involved with the fundraising coffee and cake morning on the 14th October 2017. We are glad to announce that the PPG raised £531.93 which has been presented to the surgery to help purchase a wheelchair and it's maintenance. Click here for more information and pictures.
Care Quality Commission report 2015
The CQC have issued their report following their visit on the 9/6/2015. The practice is pleased to announce that the CQC have given a "Good" overall rating for the services that we provide. Please click here or on the tab on left hand side labelled the care quality commision for the full report.

How information about you helps us to provide better care.
Information about you and the care you receive is shared in a secure system by healthcare staff to support your treatment and care.
It is important that the NHS can use this information to plan and improve services for all patients. Information from all the different places where you receive care, such as your GP, hospital and community service, will be linked to provide a full picture. This will give a comparison of the care you received in one area against the care you received in another, to show what has worked best.
Information such as your postcode and NHS number, but not your name, will be used to link your records in a secure system, so your identity is protected. Information which does not reveal your identity can then be used by others, such as researchers and those planning health services, to make sure the best care possible is provided for everyone.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please speak to practice staff.Further information is contained in the leaflet “How information about you helps us to provide better care” and the Frequently Asked Questions document.
We have added new forms to help you to help us.
Cancel Appointment Form. You can fill this in online to let us know if you no longer need an appointment you have booked. we can then cancel it and offer it to another patient.
Change of Details Form You can fill this in online to let us know if you have moved, or changed your telephone number or your name. When we receive the form, we shall update your records.Feedback Form. We would really like to know what you think of the services we provide and the way in which we treat you. Let us know on this form.
Calling All Carers
We would like to offer our carers any help that is available. So that we know you are a carer, please register at the surgery. Ask at reception for the leaflet/registration form "Information and Support for Carers".
If you are a Carer aged 40 - 74, you may be eligible for a free NHS Health Check.
If you are a carer of any age, you are entitled to a free NHS flu vaccination.

Free NHS Health Checks
These are intended to assess the risk of developing heart disease, stroke, kidney disease or diabetes. These will made available to patients aged 40 - 74 who do not have pre existing disease. They will be offered every five years. The screening clinics will be run by our practice nurse Sue. We particularly welcome Carers to have an NHS Health Check but if you would like more information on the service, please contact the surgery.
