How to register with us
We are currently accepting new patients. If you wish to register at Quantock Vale Surgery, you can either collect a new patient registration pack from reception, complete it and return to the practice. Alternatively download the following form to complete at home and return it to the surgery. It is helpful if you are able to provide proof of identification and address at the time of registration so that we can confirm your details and ensure that you are living within our practice boundary (please see below). However you will not be refused registration if you do not have this.
