Requesting Sick Notes

The surgery is introducing a new way of requesting sick notes. We are encouraging all patients to use our online platform. It allows patients to detail why they are unwell, the exact amount of time that is required, if they want a phased return to work or an extension of an existing sick note. In some case a clinician may still need to correspond with you. However this method allows a quick and efficient way for you to get a sick note without having to phone the GP surgery.
